If you value protecting the health, safety and security of America and the world and wish to make the CDC Foundation part of your legacy, one of the easiest ways to do this is to include the CDC Foundation in your will or estate plans. If you already have or are planning to include the CDC Foundation in your estate plans through a will, a trust or as a designated beneficiary, we encourage you to let us know. This way, we ensure that your legacy gift honors your philanthropic intentions, while achieving your financial and tax planning goals.

Individuals and couples, who have left the CDC Foundation in their will or other estate plans and notified the CDC Foundation of their commitment, are considered members of the Healthy Futures Society. This honorary society recognizes these individuals, their families and their legacy commitment. The generosity and foresight of these donors helps extend the lifesaving mission of the CDC Foundation and the work of the Centers for Disease Control and Prevention (CDC).

Sample Bequest Language

UNRESTRICTED GENERAL BEQUEST

I give, devise and bequeath the sum of $_______ to the National Foundation for the Centers for Disease Control and Prevention, Inc., a nonprofit organization incorporated in the State of Georgia, its principal office being located at 600 Peachtree Street NE, Suite 1000, Atlanta, Georgia 30308 for its unrestricted use.

GIFT FOR SPECIFIC PURPOSE

I give, devise and bequeath the sum of $ __ OR all my right, title, and interest in and to __________ (describe property) to support _____________ (i.e., Chronic Fatigue Syndrome research.) at the Centers for Disease Control and Prevention through the National Foundation for the Centers for Disease Control and Prevention, Inc., a nonprofit organization incorporated in the State of Georgia, its principal office being located at 600 Peachtree Street NE, Suite 1000, Atlanta, Georgia 30308. If this purpose is no longer applicable/useful, the Board of Trustees may take action to use it for other areas of need.

GIFT OF RESIDUE

I give, devise and bequeath my residuary estate, which is all the rest, residue and remainder of the property owned by me at my death, real and personal and wheresoever situated, to the National Foundation for the Centers for Disease Control and Prevention, Inc., a nonprofit organization incorporated in the State of Georgia, its principal office being located at 600 Peachtree Street NE, Suite 1000, Atlanta, Georgia 30308.

CONTINGENT GIFT OF RESIDUARY ESTATE

I devise and bequeath the residue of the property owned by me at my death, real and personal and wheresoever situated, to my wife (or husband), if she/he survives me, otherwise, to the National Foundation for the Centers for Disease Control and Prevention, Inc., a nonprofit organization incorporated in the State of Georgia.

AMOUNT OF GIFT DEPENDENT ON SIZE OF ESTATE

I give, devise and bequeath the sum of $ _____ or _____ percent of my adjusted gross estate as finally determined for federal estate tax purposes, whichever is greater (or lesser), to the National Foundation for the Centers for Disease Control and Prevention, Inc., a nonprofit organization incorporated in the State of Georgia, its principal office being located at 600 Peachtree Street NE, Suite 1000, Atlanta, Georgia 30308.
