Program Description: To improve hypertension control in metro Atlanta—a leading contributor to cardiovascular disease deaths in the region—by forging strong connections among individuals, health care providers and community organizations to promote heart-healthy behaviors, connect people with prevention services, drive improvements in health care quality and empower individuals to take steps toward better cardiovascular health.
